Introduction to my condition
Hi doctor, I was first diagnosed with gout in October 2023. That time, it was really bad—I couldn’t walk at all for about a week. After I recovered, I didn’t have any more flare-ups for a while.
Recent flare-ups
But this year, things got worse. I’ve had three flare-ups in just a few months: The first was in mid-April. I started walking again around May 6. The second happened around May 11, and it was more serious—I couldn’t really walk properly until about June 2. Then I had a third flare-up starting June 7, and it lasted until just recently. I’m walking again now, but last week I was still limping.
Imaging and previous findings
Back in 2023, when I first had gout, I got X-rays and DR scans of my left ankle. The scan showed a small round shadow—about 0.3 cm—near the ankle. The doctor wasn’t sure, but said it might be some kind of cartilage damage. Then, during the April flare-up, I had another scan and this time they found a talar spur. I’m not sure if that’s what’s causing my problems now, or if it might need surgery.
Current symptoms
Right now, I can walk, but my left ankle still feels weird, like there’s something stuck or pinched in the joint. I can’t stand on my toes, and I can’t bend or flex my ankle very far—it just feels tight and painful if I try. There’s still a bit of swelling, and sometimes I feel a dull ache or gouty twinge, especially if I’ve been walking a lot.
Medication use
I’m taking diclofenac right now. I’ve already finished two full courses that my doctor gave me before. Now I’m on a third bottle, but I don’t take it every day—only when the joint feels sore or tight, like the gout might be coming back.
Questions and concerns
So I just want to figure out what’s really going on.
- Is the talar spur making things worse?
- Do I need new scans, like an MRI, to check the cartilage or bone?
- Is surgery something I should be thinking about?
- And should I maybe start taking uric acid-lowering medication to help prevent future attacks?
